To have an existing order reviewed or changed, you would file PG 190, Petition for Review. Anyone including the protected individual can request a review of an existing guardianship or conservatorship. This should be a request for a substantial change to the order. A few examples of changes might include:
- Concern with a guardian or conservator
- The person no longer needs part or all of the protective order
- Add or remove a co-guardian
- Resign
You can attach letters or other documentation to this form to support your request.
You may want to make other requests to the court. There is now a form, PG 179 for more financial requests such as:
- Increasing a room and board authorization
- Receive a payment for services either as guardian or through a funding source (like Medicaid)
- Request fees or other payments to the guardian or conservator
Should you wish to make other changes such as moving a hearing, participate telephonically, request more time for a required filing or other requests (in which you can note and attach documentation to) you can use PG 920 as a generic form or filing your own motion and order.
